Petition Letter
Dear Dr. Stephenson,
I'm writing to petition for a late withdrawal from MICR 386 and an exam deferral for PHGY 350 to the end of the summer or to the next time the exam is offered. I have contacted both professors, my MICR 386 professor replied and indicated that I needed to file a petition with your office. I have not heard back from my PHGY 350 professor.
Over the past semester, I have been under the care of my physician for mental health issues (based on physician appointments on 23/02/23 and 24/04/23, note attached). With the help I was getting, I thought I could complete the course requirements but over the course of the semester, I struggled to keep up with the work in all of my courses and my condition affected my academic standing. By the end of the term my condition worsened and I was unable to complete the exams for the above mentioned two courses on April 15 and April 16. On the morning of April 15 I visited Hotel Dieu Hospital??s walk-in clinic, see attached note from Dr. Calder-Sprackman.
This summer, I will be coordinating with my physician and therapist on an ongoing basis to manage my condition for the fall. I will also be consulting an academic advisor to adjust my course load for future semesters.
