Subject: Petition to Return Early from RTW3 Request to Withdraw For Three Years
Petition to Waive a Requirement to Withdraw for Three Years
Petition for an Extension of an Incomplete Grade or Deferred Exam Beyond One Term
a. The Request:
I am petitioning the Faculty??s decision that I must withdraw from Queen??s for three years, and I am requesting permission to return early to full-time studies under permanent disability from the Requirement to Withdraw for Three Years.
I am also requesting that the Requirement to Withdraw for Three Years be downgraded on my academic record and waived to reflect the facts that I was completely incapacitated due to injury and reinjury, with catastrophic physical injuries and severe debilitating pain that was unforeseen yet recurring and completely beyond my control.
I am requesting an extension to all my approved IN Incomplete grades or Deferred Exams Beyond One Year ?? now turned to Fs - to have full electronic and digital access including all progress completed to date granted back for all outstanding in-progress courses with approved IN Incomplete status which then flipped to Fs automatically at the end of the next term ?? these courses need to be brought back up on onQ and LMS - all outstanding IN Incomplete courses had been documented and approved after each semester and manually carried forward with Verification of Extenuating Circumstances and signed medical documentation submitted to Disability Services each semester and reviewed annually.
I am requesting Permission to Complete All Outstanding Course Deliverables for each IN Incomplete course that changed to an F the following term and to complete any, and all, outstanding assignments and examinations while producing an earned numerical grade as my final grade to be corrected and reported on my official transcript.
b. Explanation of Extenuating Circumstances:
Jan 2019 High-dose Anti-inflammatory that I take everyday and have for years suddenly became unavailable anywhere in the world and there was no substitute. This lasted for 18 months which caused additional restrictions to my mobility.
Mar 2019 I suffered repeat incidents of walking out the front doors of John Orr Tower on west campus on my way to main campus to class when someone would blast their car horn (ie locking their car doors) or set up reverse back-up beepers and every time this happened it acted just like a shotgun blast and burst my hysterectomy incisions open on either side of my lower femoral artery causing it to split and me to be bleeding internally, instantly losing use of the left side of my body and I would then have to drag myself back into the apartment building and up to my apartment where it would then take me at least 20-30 days to recover, This happened repeatedly from Sept 2018 ?? Jan 2023.
Dec 1, 2020 My father died and because of Covid restrictions I was not informed until two weeks later and I physically collapsed as a result of instant feelings of deep emotion which caused an instant drop in blood pressure, forcing the blood to reverse through the lower femoral artery and the valve to close and the blood can not return to my heart thus causing an instant heart attack and loss of the left side of my body.
Received booster shot for Covid and suffered 3 heart attacks, 3 different mornings in a row with full loss of left side of face and left extremities.
I suffered a concussion when I stood up accidently directly under a large tree branch and the roof of my head was crushed downward. Then a week later I hit a tree head on in the dark and suffered severe whiplash.
Sept 2021 Crushed ankle when electric steel bed frame swung off dolly and crushed my left ankle. Was incapacitated and unable to walk for 6 weeks and further mobility difficulties until January 2022.
Dec 2021 Then bacterial infection, then dental infection being a repeat cycle over last two years when ODSP-Accerta Treatment Plan was not approved by Accerta and I had to wait until the expiration of the approved 5-year treatment plan, Feb 2023. Remaining teeth need to be extracted and dentures implanted due to disintegration of teeth caused by severe osteoporosis and surgically induced menopause.
2022-2023 My son came home testing positive with Covid ?? within days I too had Covid and have suffered the same incapacitating symptoms at least twice since then, knocking me right off my feet each time with same excessive fatigue and massive head pain.
May 2023 Dental surgery finally to take place after 2 additional years of suffering when 5-year Treatment Plan could not be altered or changed before the end of the existing Treatment Plan Feb 2023
May 2023 Dental Implants to now be implanted.
These injuries and reinjuries are devastating each and every time they happen, bringing my life to an immediate halt because I am unable to walk, and I suffer with severe and debilitating pain and incapacity and a complete loss of rotation and movement. All I can do is truncate all my extremities and along my spine and abdomen and hernia while staying quiet and many times being forced to go to sleep and/or remain asleep as the only reprieve from the pain and suffering.
I am under continuing care with my family physician who also is a sports medicine doctor. My doctor has witnessed the severity of my injuries and my acute and chronic pain issues and documented these extenuating circumstances each semester.
Another Major Issue
Another major issue that caused grievous harm was exam testing online with an unknown Proctor as my disability accommodations were not met. I was not allowed to speak to myself during testing, I was not allowed to read questions out loud, I was not allowed to have a piece of paper to write or work out answers on before submitting answers online,
I was not allowed to take a break during testing as is approved, as well as being able to go to the washroom when needed, to take medicine when needed, to eat food when needed in order to take medicine, and to lie on the floor and stretch if needed.
In addition, each of my Professors agreed that the online testing with unknown Proctor and 4.5-5.0 hours without a break and my disability needs not being met was unacceptable and made such testing an improper way to test my knowledge.
The testing needs to be broken down into two smaller sessions and each Professor will proctor their own exams with me.
Good News
At the beginning of March 2023, I started taking daily supplements from the Back Institute to remove the fibrin built up in my system and to repair cartilages, ligaments, tendons, joints, muscles, and cardiovascular support. For 36 years all I have done is mask the pain with pain medicine ?? other than high doses of Vitamin D for severe osteoporosis this is the first time I have taken medicinal dietary supplements to heal my internal incisions and organs, and to rebuild the damaged sheaths of my muscles and all my damaged ligament and joint structures. I have already noticed a remarkable difference with my mobility and my freedom of rotation as well as an improvement in my strength. I have been able to get my spine out of contraction for hours at a time, and I have been able to start and maintain a routine exercise program.
Also, while in Kingston last week, and still severely affected by the horn blasts, I was dropped to the ground 9 times in a row because of horn blasts, however my herniated hysterectomy incisions and lower femoral artery did not burst open instantly resulting in a total loss of structure and control of the left side of my body, instead with a smile, I was able to stand back upright a lmost immediately, and carry on my way, without the total loss of my extremities on the left side of my body.
I am hoping that these daily supplements will finally help with all my joints, ligaments, heal my incisions and my affected organs and stop them from tearing apart (repairing Swiss cheese effect).
I am also hoping that I will be able to retrain my body back to good physical and mental health, with the strength and energy and optimism of being an elite athlete once again, while studying and completing my courses full-time under permanent disability.
Starting courses this semester, summer 2023, May 1st, I will take the 40% course load (normally, I have taken 60% and 100% of a course load so that I could keep up and which I did do). For the three years Sept 2018 - Aug 31, 2021, I did reach 30 courses, completing 14 courses with a grade, and 16 which turned to Fs and need to be completed,
As for the IN Incomplete courses that flipped to Fs at the semester change, I will complete them one at a time, most courses just need the final exam written, or the final assignment submitted, the following is what is required to complete these 16 courses:

My Professors will be glad to have these Fs (INs) removed from their records also. Exam taking needs to be split into 2 shorter sessions rather than 4.5 ?? 5.0 hours straight.
Upon completion of all of my outstanding courses combined with full-time studies under permanent disability, either 40% or 60% course load each semester, I will achieve the following:
