Hi,
I am petitioning for a CR in the course BCHM 218 I took in Fall 2022. This is due to extenuating circumstances beyond my control regarding medical issues and my compromised mental health. During Fall 2022, I suffered immensely from a medical issue that impacted my academic ability and capacity. This medical issue occurred during the 2022-2023 academic year. However, this medical issue was first documented on December 11th, 2022, as shown in the supporting documentation below. This medical issue caused immense distress from symptoms like vertigo, dizziness, nausea, migraine, and vision impairment. This medical issue also induced and heightened my already existing mental health disorder, causing me to experience even more damage to my health like panic attacks, vomiting episodes, hyperventilation and major fatigue.
At the beginning of 2023, this medical issue was at its peak. I was scheduled to write my final exam at the beginning of January. However, I was physically incapable of doing so due to the heightened severity of my symptoms, like visual impairment and vertigo. Due to my physician's long wait times, this health issue was documented again on January 15th, 2023. Throughout the 2022-2023 academic year, both my physical and mental health were majorly declining. I felt extremely lost, depressed and defeated from the lack of available resources. My doctor recognized the severity of my medical issue and its symptoms and signed off on the
