To whom it may concern,
Halfway through the winter semester of 2023, I experienced a significant decline in my health and had to leave school for an extended period. I received an accommodation as well as an 8-month extension on my courses as I have only now been able to return to school. I have been conversing with my professors as well as my accommodation advisor, and they have all agreed to work with me in submitting all the required material by the end of the 2023 fall term. Therefore, I am submitting a request to have my academic records reflect such.
