Petition Letter
To whom it may concern,
I am writing to request that an alteration be made to my official transcript. The request includes removing the academic record(s) on my transcript from the fall semester of 2023. During this semester, I was struggling with undiagnosed celiac disease, an autoimmune condition, which led to a sharp decline in my mental health and academic performance. I was severely ill; consistently in and out of hospital, being picked up by ambulance almost weekly, which led to the unfortunate circumstances of poor mental health and an inability to complete coursework. This led to me dropping out of classes in January 2024, at the beginning of the winter semester. Shortly after I discontinued my studies at Queen’s, I attended a 50-day inpatient program at Bellwood Health Services in Toronto. Attached below are the dates and my certificate of completion from the program. I have now moved to Vancouver, BC and hope to continue my studies at UBC. To note, I have been working with a psychologist, Dr. Huzur Altay, with whom I have been in treatment since 2019. We will be continuing sessions for the foreseeable future.
