Petition Letter
Dear Sir or Madam:
I would like to apply for an Aegrotat Standing appeal in the course HLTH 334. I have been suffering from depression and anxiety intermittently over the last four years and this past winter term my illnesses worsened significantly.
With the support of my family doctor, Dr.Lorie White, my previous psychiatrist Dr.Lynda Molleken, and my current psychiatrist Dr.Brian Macdonald, I was able to defer my final exams.
By mid-June, Dr.Macdonald and I agreed that my treatment was going well enough for me to attempt to write my final exams.
During my HLTH 334 exam I suffered a severe anxiety attack and was unable to function for 1.5 hours, about half of the given time. As a result I have received a C+ on the exam. In addition to the final exam I have been attempting to complete an outstanding assignment for the last five months. Due to ongoing impaired concentration and academic functioning, I have yet to do so successfully.
The SKHS Undergraduate Coordinator, Anna van der Meulen, has advised me to appeal for Aegrotat Standing because I have completed over 60 of the course work and my ability to perform on both the final exam and in writing my final essay have been significantly impacted by my illnesses.
If granted this appeal, I will receive a B- as my final mark. I believe this is the best course of action for me because I am fairly confident that if I submit the final assignment my grade in the course will only decline.
I will attach documentation from each of the listed doctors confirming my illness. The notes were written over the course of the spring and early summer. In addition, I have included a very recent note written by Dr.Macdonald confirming my illness and requesting that accommodations be made.
I appreciate your consideration of my request and will be awaiting your decision.
