Petition Letter
Hello Associate Dean,
I am asking for permission to request a retroactive Letter of Permission. I enrolled in CHEM 217 in July 2013, or Summer 2013 however, my Letter of Permission for this course was only valid for Fall/Winter 2013/2014. I requested my transcript to be sent from Athabasca University to Queens University. Karen McBride received the transcript and notified me that the Letter of permission did not align with the time I took the course. She requested that I notify you of this issue and explain why this mistake was made; It was two years ago when the request took place so my memory of it is quiet vague. I was told by the Environmental Science office to go to the Arts and Science office in the MacCorey building to request an application for the letter of permission. I assume that I would have explained what I was looking for to the person helping me at the front desk. I was given a application form to fill out and I completed it. I may have been given an application that was for the wrong term or it may have been me who made the mistake by requesting for Fall/Winter 2013/2014 instead of Summer 2013 on the form that I filled out. I am asking if you could change my Letter of permission from Fall/Winter 2013/2014 to Summer 2013 which would align with the time at which I took the online course through Athabasca University. I apologize for this inconvenience as I should have been more aware of this problem earlier when receiving my Letter of Permission in June of 2013.
