Petition Letter
To all those this may concern,
I am writing to you to request a further deferral of my midterm exam for PHAR 370. Unfortunately, an unfortunate event has come up, and I am going to require emergency dental surgery on November 5th. For this particular kind of surgery, I have been informed that the recovery process is quite extensive, and I even have supporting documentation from the dentist demanding that I have up to at least a week to recover. Unfortunately, I won't be able to attend my deferred midterm exam because of the aforementioned reasons. I will contact my instructor and follow up to ensure I complete the exam after I have recovered. I am very grateful for your time and consideration of my request, and for the opportunity to present my circumstances. Thank you for taking all of this into account.
