Dear Dr. Naaman,
I hope you are doing well.
My name is Phillip Stuart, and I am a second-year undergraduate student at Queen’s University within the Chemistry Department.
I am writing to you with regards to requesting permission to drop CHEM 213 for the Fall 2025 academic term due to severe extenuating circumstances that are significantly affecting my academic performance.
Throughout this term, I have been dealing with psychological difficulties that have ultimately led to my diagnosis of bipolar disorder. Such difficulties started in September and have progressively worsened over the course of the semester.
My circumstances have, and continue to, impact my academic performance in CHEM 213, particularly within the lab components and in-class tests. During the span of this semester, I have been unable to consistently attend lecture or designate adequate preparation for assignments as a result of my psychological state. Despite my best effort, I am unable to keep up with the class and perform at my optimal academic level.
I continue to seek medical and professional help for my condition. Following admission to the emergency department, I was designated a psychiatrist who formally diagnosed me with BPD. Subsequently, I was also connected to a psychologist who has been helping me take the initial and necessary steps in stabilizing my lifestyle, health, and mental well-being so that I can begin to pivot back to my academic responsibilities.
Please consult the supplemented supporting documentation from my psychiatrist and psychologist to verify the nature of the aforementioned circumstances.
