To whom it may concern,
As a result of mental health issues which I was facing, I have decided to withdraw from school temporarily. I was not able to complete these courses because of the mental health issues I was facing.
My intention is to resume studies in the fall of 2016. Currently, I am receiving treatment in Toronto.
I would like to apply for the following: an aegrotat grade in BIOL 319 and KNPE 355, and an extension of my incomplete grade in CHEM 282.
I have attached a letter from my family doctor explaining the current circumstances, and I have submitted previous documentation regarding my condition to the school.
