I have been suffering from severe depression for the last several years. As a result, I was not able to complete all of my courses in the Winter 2015 semester and failed to appeal these grades in time.
My mental health issues persisted through the summer and fall of 2015. After the end of the Fall 2015 semester, I returned home to my family and asked for their support.
Now, my situation has improved dramatically, I am seeing a psychologist on a weekly basis and I have received accommodations from Accessibility Services.
Please find attached letters from my family doctor and a psychoeducational assessment.
