Petition Letter
To whom it may concern,
This letter is to request Aegrotat Standing in GRMN 101, taken in Winter 2016.
It is vital for you to know that I have struggled with my psychological health for some years now and that, although I have actively sought out help many times and I work consistently to treat my mental health and mitigate the symptoms of my metal illness, I am still faced with recurring relapses of my mental illness (see notation at the bottom of the page). This is pertinent to this request, as two recent mental illness relapses over the last five months have affected my ability to write my final exam for GRMN 101.
To be more exact, during the April 2016 final exam season, a combination of both a physical illness and the relapse of my mental illness resulted in the deferral of two exams, including that of GRMN 101, as recommended by a counsellor at Queen's Counselling Services. During the last month and a half, I began to relapse back into the symptoms of my mental illness, which unfortunately happened to coincide with the weeks coming up to my scheduled write date for my GRMN 101 final exam (September 8th). The effects of this relapse consequently led to the advisement and recommendation from Janet Roloson, a senior counsellor at Queen's Counselling Services, to defer my final exam to a later date again.
Because my challenges with my psychological health are ongoing, my instructor has recommended that I make this appeal. As it is unknown when or if I will be able to write the final exam in the near or distant future and if this situation may occur again, we believe that Aegrotat Standing is appropriate in this situation. We also recognize that, as more time passes, it will become much harder for me to perform well on my exam and display my true understanding of the material.
Please contact me if you require any more information or insight into these circumstances and how they have affected my ability to write my final exam, from myself or in written form from a counsellor at Queen's Counselling services. If you would like more information about my plans to deal with my mental health issues moving forward, or if you would like the copies of the Request for Special Short-Term Academic Accommodation forms that I received from Queen's Counselling Services during the April 2016 exam season and on September 8, 2016, please feel free to contact me as well.
Thank you for your consideration of my appeal,
Lily Helmer-Smith

Note: When I state my mental illness, I do not currently have supporting documentation with a diagnosis of a mental illness. I will be attending a mental health assessment by a physician on September 19th that may result in a diagnosis of this nature. If this is required for my appeal, please contact me for more information. In the context of this letter, I say my mental illness to refer to the ongoing and recurring symptoms and challenges that I face with my psychological health.
