Petition Letter
Dear Associate Dean of Arts and Science:
I hope this letter finds you well. My name is Katie Haas and I am currently in my 5th year at Queen's University majoring in Art History. I am writing to you today in hopes to have the 21-day appeal deadline waived for the following courses:
o MATH 126 - Taken in the academic year of 2012/2013
o CLSTS 201 - Taken in the academic year of 2015/2016
In regard to MATH 126, I did not submit an appeal within the 21-day appeal deadline as I was struggling with mental health issues as a result of my father's sudden death in February 2011. Upon entering university I was struggling with my mental health and experienced extreme forms of depression, for which I eventually sought out counseling at the Lasalle building on Queen's Campus. The year proved to be very difficult for me mentally and academically as a result of my depression. Attached to this appeal letter is documentation of my visits with counselors and doctors at Lasalle during the academic year of 2012/2013.
In regard to CLSTS 201, I did not submit an appeal within the 21-day appeal deadline as I was still struggling with health issues that developed in November 2015. I was initially admitted to Kingston General Hospital for Pyelonephritis (kidney infection) and later developed a Pulmonary Embolism for which I was transferred to the Intensive Care Unit. Following my discharge from KGH I received outpatient treatment intravenously through a PICC line for duration of 10 days, as well as routine follow-up appointments. Attached to this appeal letter is documentation from Kingston General Hospital outlining my condition and series of events that took place during my admittance at the hospital.
As you can see, there were severe extenuating circumstances that prevented me from submitting an appeal for both MATH 126 and CLSTS 201 within the required 21-day appeal deadlines, and it is my hope with this letter that the deadlines be waived so that I can move forward with my appeals.
