Submit an Academic Petition or Appeal

I am appealing to request an extension on two incomplete grades in the fall semester of 2015 for PHIL 261, Philosophy of Mathematics, and PHIL 510, an independent study, which have since been changed to failing grades. For reasons previously documented in an earlier appeal due to the physical and mental trauma of a sexual assault, the course work for these two courses could not be completed on time. I am looking to submit the work so that I may move on from this.
