Dear Sir or Madam,
I am writing to you to request MUSC 160 A and MUSC 160 B, Choral Ensemble, please be removed from my transcript. In September of 2016, I attended the Choral Ensemble audition and then decided not to participate in the course, choosing to be part of MUSC 162, Symphony Orchestra, instead. Upon this decision I emailed Darrell Bryan, choral director, to notify him I would not be taking MUSC 160 this year. He replied in an email, acknowledging my decision (see email 1 below). In February, I noticed MUSC 160 was on my transcript and immediately emailed Kim Gudlauski, Dan School of Drama and Music Secretary and Auditions Co-ordinator, to bring this error to her attention (see email 2 below). In hindsight I should have followed up with Mrs. Gudlauski but I made the mistake of just assuming MUSC 160 would be removed from my transcript. It should be noted that throughout the entire academic year I did not attend any choral ensembles, participate in any tests midterms or exams. I was never a recipient of emails from the choral director either beyond our initial communication. Today, May 2, 2016, I have spoken with Mrs. Gudlauski several times regarding the MUSC 160 and the grade evaluation of an F that appears on my transcript. Mrs. Gudlauski reported to me that back in February she thought Dr. Stephanie Lind, Chair of Undergraduate Studies, had replied to my email and Dr. Lind thought Mrs. Gudlauski had done the same. Needless to say, neither replied, thus, MUSC 160 was not removed and I received an F for a course I did not take. This miscommunication has resulted in me submitting an appeal.
I hope this situation can be resolved swiftly, and that MUSC 160 can be removed from my transcript along with its grade. Thank you for attending to this matter.

I have chosen to be part of the symphonic orchestra this year and plan on taking part in the choir next year. Sorry for the late notice; it was a late decision on my part.

Hey Kim,
I was just looking on my SOLUS to see if I could find my grades from last semester and noticed that I'm enrolled in choral ensemble and not in orchestra, which should be the other way around. I know I don't have the authorization to change that, but I was wondering if you did, if you wouldn't mind looking into that for me when you get a chance.
