Dear Dr. Benard:
Re: Charles St. Louis-MATH 337, request to have a courses grade listed as CR.
I am appealing to mark my MATH 337 D + mark as CR due to a serious medical condition I experienced this past term.
At the end of January, I was hospitalized with bacterial meningitis (meningococcal B strain) and suffered complications (as a result of the treatment) with a drop in my oxygen levels (see attached hospital discharge papers). I was in the ICU for 4 days and was on intravenous antibiotics for 14 days. As a result of this condition and complications, I was only able to return to Kingston and to class on March 2, 2017. On my return, I was not functioning at my normal capacity. I suffered frequently from headaches and had difficulty concentrating. These health issues, combined with having missed so much class time, made it difficult for me to catch up in this course and hence my grade suffered greatly.
