Dear Associate Dean of Studies,
I am appealing to drop a course from my academic year, CHEM 281 (Summer 2014). Despite it being past the due date to drop courses, I am asking for permission to drop this course due to my medical circumstances. I am also asking to extend my IN grade for MATH 121 (Fall/Winter 2013/4) and BIOL 103 (Winter 2014) due to my medical issues this summer.
I have been struggling with a physical illness that has been affecting my academics. This has brought a fair bit academic dilemma and a large dropping of courses. I was diagnosed with a fairly large ovarian cyst in the summer of 2013 and thereby bringing in exhaustion, and low energy with me to this academic year and into the summer. Symptoms include harsh abdominal pain, low energy and exhaustion. Originally my plan was to take CHEM 281 again in the summer and finish my incomplete courses of BIOL 103 and MATH 121 so that I don't fall too behind in my plan. This was also what I had discussed with my academic counselor as a plan. I had my summer planned to write the exams before my surgery on early July, undergo the surgery which was booked for July 23rd and be fully recovered in time for school in September. However due to my medical circumstances on the day of CHEM 281 exam I experienced pains and was forced to miss my exam, failing the summer course as a result. The pains led on into early July resulting in the inability to write my incomplete course exams and I went into emergency again on July 7. My cyst had become very severe and I had a large surgery the night of July 7. Unfortunately no one had an idea as to how large cyst had become and how twisted it was in my system or I would have been more prepared for my circumstances. I am now finishing recovery but do realize that my courses are to be taken care of.
In terms of future plans, I will be taking the Chemistry course in the year as I am now finishing recovery of my disability. The extension of the BIOL 103 and MATH 121 will help me study for the exams so that I can take them late September/early October before midterm season hits. I will continue to see a medical professional for my emotional distress and continue to get support. Once I am caught up with the two incomplete courses in a month, I will be taking a healthy course load and will not have to worry about medical conditions interrupting my academics as well. I will mostly likely consult with an academic advisor once more to make sure my path of courses is correct and is not askew in terms of graduating. I have discussed with an advisor that I will stay at Queen's for an extra semester in order to graduate if needed.
I have two documents supporting and proving my medical events. One states that I was unwell on the day of the exam of my summer course as I mentioned previously. The other shows that I underwent surgery and was in recovery until very recently.
Please consider my request as I wish to continue my academic year in a healthy manner now that I am well. I hope to finally study during the year without interruptions and I am sure that dropping the summer course due to my medical issue and extending my incomplete courses to have time to study and write my exams will help relieve stress as I start the new academic year.
