Petition Letter
To whom it may concern,
I would like to appeal to waive the 21-day deadline regarding my completion of my PACT 333 course, which was originally registered in the Winter 2016 semester. During the semester of Fall 2015, I had suffered a herniated disc in my L4 region of my spine. This prevented me from participating in physical activity for the remainder of the year, as I often struggled to even walk to class. PACT 333 requires 30 hours of physical activity with a person with a disability. Because of my inability to participate, it was agreed with my instructor, Professor Kathy Jackson, that I was to complete these hours in the next year (starting in the Fall 2016 semester). I had finished all of the course requirements in March of 2017, and my change of grade was then submitted, and approved by the department.
I was not notified until 2 days ago (July 7, 2017) that I needed to file an appeal form to extend this course for the year, and therefore was unaware that an Incomplete (IN) grade was to completed in the following semester (which would have been the Summer 2016 semester, and would then be changed to an F on September 15, 2016). Because of this, I had not submitted an appeal for this grade.
