To: Associate Dean of Arts and Science
I am appealing my final grades in Chem 281 and 285 that was taken during the
Spring/Summer term of 2017. I am asking to be granted a CR credit for both courses due to
unforeseen circumstances that were beyond my control as indicated below.
The basis for my appeal is due to an illness and operation that I required in May 2017.
The extreme urgency of this operation and circumstances relating to this operation left me in a
state of distress to which I have now come to terms with through counseling. The procedure
was a dilation and curettage (D&C) following a miscarriage.
With the increased stress and my prior learning disability it became very difficult to focus
on both exams which cause a normal level of stress, and due to my emotional disability the increased
anxiety and stress triggers my dissociation disorder. Both exams weigh 80 of the final grade
and require a pass of the exam in order to pass the course. I have now registered with
accessibility services at Queen¿¿s to ensure my accommodations are in place now prior to future
exams and have sought counseling to discuss my emotional health. Please see required documents to assist in understanding my learning disability and the
operative form that I received on discharge from the hospital.
With assistance through the Arts and Science department and accessibility services I
hope to receive the ability to continue my studies and maintain the same schedule in hopes of
graduating the spring term of 2018 with a BSc Gen in Life Science.
