Petition Letter
To Whom It May Concern,
My health situation last semester severely impaired me from performing in school. For this very reason, I request that the following three courses (ECON255, ECON 232 and STAT 263) be dropped from my transcript, so they do not impact my future educational goals and do not penalize me for the circumstances beyond my control.
My name is Usaid Razakazee, and I am a Queen¿¿s University student who should be going into his third year. However, due to extenuating circumstances, I was unable to complete my second year.
During my last semester, I got severely depressed; usually waking up in a low mood and finding it very difficult to carry on with activities that I usually found enjoyable. With a loss of motivation to live; school and my social life became inconsequential.
Since November 2017, I was struggling with daily chores that lead me to consult a psychiatrist
in Markham. At that time my diagnosis was unclear, and I continued to suffer in silence. However, in April 2018, as a result of my mental breakdown, I came to know what I have been suffering is attributed to the bipolar disorder. Since then, I have been working closely with the doctors on getting back to my normal mental health. I am expected to be fully functional by August 2018 allowing me to get back into the mix.
Although I was able to submit my assignments and quizzes, course work was usually incomplete and not at par with what was expected. Moreover, towards the end of the semester, I suffered a mental breakdown which resulted in getting me checked into the psych ward at Kingston General Hospital on April 18, 2018.
I am attaching the following documents for your easy perusal:
¿¿ Request for Academic Consideration for Extenuating Circumstances; and
¿¿ Verification of Personal Health Condition.
I thank you in advance for helping me get back on track with my studies. Please feel free to let me know if you require any additional information that would help facilitate my request of removing these courses from my transcript.
