Petition Letter
Dear Associate Dean of Studies,
I am writing to appeal to drop FILM 240 Fall 2014. I have been very sick since Tuesday, October 28, 2014 and unable to attend classes or do any readings or other course work. I have been suffering from nausea, vomiting and weakness in the mornings for most of the term, but it has recently developed into all day, unbearable symptoms.
I saw my family doctor twice this fall. I went to Emergency at Hotel Dieu on Saturday, where I was given intravenous fluids and anti-nausea medication. The doctor also gave me the attached medical note. They told me to come back if it gets worse, but I'm not improving and I am fearful of the impact it is having on my classes and coursework. I also suffer from anxiety and depression , which is being treated, but as a result of my physical illness my mental health is suffering. I had an appointment at Health, Counselling and Disability Services this morning and I am attaching the letter of accommodation they gave me. I also sent a copy to each of my professors and am making them aware of the situation.
My primary concern now is being behind in my courses; I am arranging for appropriate extensions where needed, and while Dr. Matrix has offered me accommodation, I feel the best course of action would be dropping FILM 240 to allow me to focus on the courses directly related to my major, and preforming to the best of my ability in those.
