Petition Letter
To whom this may concern,
I am submitting this appeal in regards to extending an incomplete grade for the course HIST 329 in which I was enrolled during the Fall/Winter of last year with Sandra den Otter.
Over the past couple of years, I have struggled immensely with my mental health and was unable to fulfill required tasks such as attending class and submitting assignments. I was placed on academic probation and was told that I was going to have to withdraw for a year. Over the summer (of 2018), I successfully appealed this requirement with the help and support of my family doctor and one of my current counsellors. I have received extensive support and have made tremendous progress which is being reflected in my current academics. I will include a letter from my family doctor, and one from my personal counsellor again with this appeal. They were both written over the summer and pertain to my status while I was enrolled in Dr. den Otter's course.
It is unfortunate and stressful that I must once again submit an appeal in regards to my academics for 2017/2018, but this course is a pre-requisite and I must obtain the credits in order to successfully complete my program. I have received the additional support of a counsellor (Farrah) through the Queen's Wellness services and will ask her for an additionally letter as well. She influenced and strongly encouraged me to apply for this appeal. In order for me to enrol in 400 level courses, it is essential that I have this 300 level course completed. Since I am in my fourth year (and plan to take a fifth year), it is important that I being enrolling in 400 level courses as soon as possible - it is crucial for my graduation that I receive a complete grade.
I have already consulted Dr. den Otter regarding my appeal, and she has been extremely encouraging and willing to support me in this request.
