To whom it may concern,
During the course of the 2017-2018 and 2018-2019 academic year I suffered involuntary
set-backs due to medical reasons. I have attached supporting documents from my family
physician for your review.
I struggled at times through the fall semester and sought ongoing medical attention
during this time period and most recently in March of 2019.
In keeping with the advice of my academic advisors and family physician I have made
the difficult decision to withdrawl from this class due to my medical condition.
I ask for your consideration that, due to circumstances beyond my reasonable control, I
am unable to complete Music History MUTH111 in the 2018-2019 academic year.
In this appeal I am asking for your support to resume study in this course in the 2019-
2020 academic year and to not have my immediate withdrawal negatively affect my academic
records.
I have been supported and encouraged by my physician, my family and academic
advisors to place my wellbeing first and ask that you see this request in this light as well.
I look forward to continuing my passion and my profession in music at Queens
University.
