Petition Letter
Dear Appeals Coordinator,
I am submitting a formal appeal to this board because I am currently in poor academic standing in my first year beginner Spanish course (SPAN P10). Throughout the semester, I put in a strong effort into this class, however in the last three weeks of the semester, my grades and attendance fell dramatically. I believe that my situation qualifies as extenuating circumstances and will allow for leniency. During the majority of this semester, I was experiencing several mental health issues that worsened greatly after midterm examinations. On the 19th of November 2014, long past when I should have gone, I made a visit to HCDS and saw psychologist (Dr. Mike Condra). Dr. Condra sent me to Kingston General Hospital for an emergency psychiatric evaluation. From KGH, I was sent to Hotel Dieu Hospital for further psychiatric evaluation. After my second evaluation, I was admitted for one week at KGH. I am currently on new medications that are helping in a variety of ways. I look forward to getting back in the rhythm of university life and courses. I hope the Board of Appeals grants my request for special consideration in this matter, as I do not want this one event to set back my academic career. If documentation is required, I have a medical note from Dr. Condra and I am able to obtain another medical note from my psychiatrists at KGH. I would be more than happy to explain the situation in greater detail in person if you feel you do not have enough information to come to a decision.
