Petition Letter
Dear Professor,
This is the appeal requesting to drop Math 474 in 2019 fall term.
I am having a chronic disease (Poly Cystic Ovary Syndrome) lasting for two years. It comes with irregular periods usually at the beginning of every month. The stomach and pelvic pain which seriously affected my daily life and study, including the midterm on Nov 5th, when it exceeds the deadline to drop the course.
So I planned to focus on my final exam, but I experienced an acute diarrhea caused by PCOS in the final week and thanks for professors approving the consideration to defer the final. Unfortunately, I stayed in the hospital for that during the winter break, and the unstable health situation of entire term leaves me insufficient time to study this course.
Sorry for inconvenience. I have tried my best to eliminate the effect of this chronic disease on my study. Thank you for your understanding.
PS: The ultrasound scan and certificate of diagnosis is translated and verified by Helen Lin, (403)714-7888, License Practical Nurse at Foothill Medical Center
Sincerely,
Simin Zhang
