Petition Letter
I am appealing the Academic Year of 2019 to 2020.
I was hospitalized in Kingston then in Ottawa from November 16, 2019 to December 12, 2019 due to mental health reasons. I was unable to write my exams for Math 120, Math 110, and Phys 104. Because of ongoing issues with my mental health and on the advice of Dr. Freire, I did not continue with my academic year.
I suffered and continue to suffer from acute mental illness, which includes depression and anxiety. I am only now able to write the appeal. Because of these circumstances I would ask that any incomplete be removed from my transcript for the Academic Year 2019/2020.
If you require additional information, I have given Dr. Freire consent to speak to the University on my behalf.
I am also happy to provide you with any information you may need when considering my appeal.
