Petition Letter
Hello,
I am hoping to appeal an assignment I missed during the month of December (I believe), for an end-of-term survey which was 20 of my grade. I had accommodations from December 7th to December 31, for which I had a doctor's note. I attempted to ask my instructor about this assignment and I have yet to receive an answer as to when it was due. I asked to review this because I did not see the assignment in the timeline during the month of December and was suffering from circumstances (psychological challenges from the side effects of missing my prescribed medication for this health issue) that would have prevented me from completing this assignment.
