Petition Letter
I was diagnosed with a severe depression episode and anxiety state in 2021 and I had a relapse in NOV.
I was trapped in a poor mood, anxiety, and poor overall condition accompanied by insomnia decreased concentration and decreased energy. I was in bad shape at night and had trouble falling asleep. I was inattentive, down, unhappy. I lost interest, lost appetite, upset, and disordered schedule. I was neurotic, has negative thoughts, gets out of control because of trifles.
In this period, I had several attempts of suicide and experienced impulsive behavior, venting behavior, and self-hurting actions. Managing my time and work became extremely hard. I also had difficulties managing stress and communicating with others.
Impacted by these mental illnesses and their functional restrictions. I could not steadily participate in regular academic activities or well complete the course work. I had poor attention and memory and has difficulties in dealing with problems, which affects the completion of courses and assignments on time.
Though I had been taking the medical care from my doctor, taking Agomelatine tablets (2/day) and lorazepam tablets (2/day) at maximum dosage, my performance was strictly affected during the last term, thus, I want to make a CR request for my course math384.
