Submit an Academic Petition or Appeal

Attached to my appeal is a letter containing the following: the request, explanation of extenuating circumstances, future plan, description of courses for appeal, attachments, medical information, and medications.

I am seeking to appeal all of the courses listed on my letter. This online form would not allow me to include all of them.
