Petition Letter
Dear Dr. Jenn Stephenson,
I am sending this petition letter requesting to drop the courses ASTR 101, ASTR 102, and FILM 340, therefore changing the grade(s) to a DR on my transcript. This is based on the extenuating circumstances I had during the time of these courses in a mental health crisis and suicidal ideations. Last term I experienced a depressive episode in which I stopped all of my activity, which included my schoolwork and very important finals that I missed.
This can be seen in my December 27, 2022 visit with my doctor back home shortly after where it states under active problems, ??Encounter for screening for depression, Active?, and, ??Episode of recurrent major depressive disorder, Active?. Similar statements can be seen in the doctor's notes for a visitation previously on August 9, 2022, as I planned to leave colorado and stay at queens for the term where depression is listed under active problems. It can also be seen I have and am currently being prescribed different antidepressants and medication for manic/depressive episodes such as vraylar even back in a visit summary I??ve also included from May 7, 2021. It states, among other notes, ??He scores elevated on several aspects of the mood disorder, concern for bipolar depression?. Currently, I am taking Wellbutrin XL 300mg tablets every day, which can be seen listed under current medications in the most recent visit summary of December 27, 2022. Of course, as a result of my decline in mental health, I received an F in all my Fall 2022 courses on my transcript. Moving forward, I have realized that biology is not the career path for me, I don??t enjoy the subject and that prevents me from doing my best, especially during times of mental crisis, so I will be ideally switching programs to film and media this May. To prevent this from happening in the future, I am not only going to be attending counseling but also taking a term away to focus on my mental health and prepare myself to return after the summer to a new program. This along with continued use of counseling and medication is my future plan for achieving my academic goals.
