My name is Ryan Inayeh, student number 05807231, and I wish to appeal the grade that I received in MATH 474 in the fall 2014 semester for credit (CR) standing. I have spoken to an academic adviser and they have the required documents.
On November 11 2014, I was admitted into the emergency room of the Kingston General Hospital for a severe kind of pneumonia called pneumocystis pneumonia. I received a heart transplant last April and as a result I now have a compromised immune system. Due to my weak immune system I got infected with the pneumonia. I was at the KGH for two days until I was transferred to the Ottawa Heart Institute on November 13, where I was hospitalized for about 3 weeks then sent home in Ottawa to recuperate. Student services already has a doctor¿¿s note from the Ottawa Heart Institute; I submitted it to Spring Forsberg. I have already discussed this with the department, particularly associate dean Dr, Johanne Benard, and both Disability Services and your department should have documents as well as records of emails I've sent describing the situation.
As a result of the hospital stay, I missed exam period and was forced to take an IN in my courses. I still have not written the exams for two of my courses, but I wrote one exam in January for MATH 474 when I was still recovering but able to come back to school. I also wrote the midterm exam for MATH 474 on November 4 2014, when I had the infection but I did not it. At the time it felt like a really bad cold, but I was struggling to walk to campus. I wrote the midterm when I was sick and the final when I was recovering, and I did more poorly then I probably would have. I believe that I am qualified to appeal for CR standing in that course for that reason.
I have already mentioned this to an academic adviser, Spring Forsberg, and she said that I have a legitimate claim for CR standing. The only thing I was concerned about was the 21-day submission period for appeals. I wrote my exam in January, but I did not think about the course until the rest of winter my semester was done. She said that the 21 days was just so the appeals are still only for recent courses, and that there is some leeway for it.
I hope this sufficiently covers the necessary grounds for an appeal. Please contact me if you require other documents that Disability Services does not have.
