To whom it may concern,
My name is Danica Wall, and I am writing to appeal to late drop failed courses due to a flare up in my disability. I am hoping to drop CHEM223, ENSC390, DEVS221, and BCHM218 in 2021 Fall and 2022 Winter and to receive an incomplete for CHEM497 in Winter 2021 so I can finish my final project for this course during the summer term as discussed with my supervisor.
I am also hoping to drop DEVS221 in the Winter 2023 term. I am slowly transitioning to get back into finishing my degree but have had some difficulties adjusting which has caused me to be unable to finish all the courses I have hoped to this semester.
I have been struggling with mental illness over the last couple years. During covid it became unmanageable, and I spent many days in and out of the hospital. Unfortunately, this made it extremely difficult for me to attend class, complete assignments, and write exams. I had to stop attending class due to my admittance to the hospital and difficult circumstances that lead to failing courses over the last couple years while trying to finish my final year.
I know I should have communicated earlier and taken care of a leave of absence but at the time I was unable to do so.
It would mean a lot to me to be able to complete my degree as I have made it this far and do not want my disability to affect my future so negatively and know I am ready for the challenge of finishing school. I have registered with QSAS and will be following up regularly with my doctor and psychiatrist with a mixture of medication and therapy to manage my illness.
If any further documentation is needed, please do not hesitate to contact me so that I can provide any additional documentation that would be required for a successful appeal, such as a doctor's note or note from my counsellor. I thank you in advance for your consideration.
