Submit an Academic Petition or Appeal

Petition Letter: On Saturday April 15th, while writing my Macro Economics exam on campus in class I experienced a seizure. This resulted in the need to have paramedics pick me up from campus where I was writing my exam to transport me to Kingston General Hospital where I underwent a number of tests including a CT Scan, ECG and blood panel. I have since been released, however, it was recommended that I see a physician in Toronto to undergo further testing. I am currently back home in Toronto and am in the midst of having a full work-up of tests done, including an MRI and EEG to name a few. I am not to be left alone and as such, the safest place for me to be right now is at home.

Given the above circumstances, I??m am writing to you in an effort to determine the path forward for my exam and course completion. I??d like to ask that I be given an exception to not be required to complete my exam and that my term average be given as my final grade. My health is my biggest priority right now and I ask for your consideration. If there is somebody else I need to speak to about this, please let me know.

I am happy to forward the attending physician's statement from Kingston General along with the discharge forms for your review.
