Submit an Academic Petition or Appeal

Petition Letter: I am requesting on term work for the following courses. I have really been struggling with my severe anxiety and panic disorder for the last year or so, but it began unbearable since April. I have been struggling with everyday life, which severely impacts my academic life. Due to all the assignments due in end of March and all of April, it has increased the negative affects the disorders have on my body. When my anxiety disorders our, I get physical symptoms that make everyday activities unbearable ( migraines, nausea, dizziness, chest pain and rising heart). I have been in contact with Dr. Fizzle, and the TA in Glph 471/ about my
Struggles but have not told my other professors ( I was afraid to tell them because I thought they wouldn??t care). My case worker angel is aware that my struggles are impacting my studies. My family doctor is able to send in some paper work if needed, they are aware of my disorders and we are trying to manage with medication ( but we are having issues finding a medication that works. I really need these courses, I am taking 3 courses this summer term and taking 2 courses this fall term and hopefully graduating then, I really need these courses, this degree has severely impacted my anxiety disorders and I am becoming burnout, which is impacting my mental health even more. Please accept my appeal, I am able to answer any questions needed.
