Petition Letter: Hello,
The class that I am requesting to join is Math 121 over the summer semester. The reason for this request is that I need to retake this class as the grade has just been posted and I have failed. The reason for me failing this class is due to severe medical conditions that I had throughout the semester and during my exam. I had bacterial pneumonia while studying and writing this exam. Throughout the semester I also had a stomach bacterial infection that resulted in many missed classes and an inability to adequately complete assignments. I have documents that show proof of these medical conditions and can submit them if required. Over the past few weeks, I have gotten treatment and recovered from both conditions. I understand that it was by mistake for writing the exam even though I had approved academic consideration. The reason that I forced myself to take this exam was because I believed that writing all of my exams during the deferred period would not be a good decision as there are 4 days of deferred period and I would have to write 4 exams. I believe that if i were to retake this course with my current recovered state, I would be able to achieve a much better overall grade.

Academic Plan
Academic Plan: My academic plan for the future is to stick to a strict study plan that I had set in place before getting sick. My plan involved a schedule of daily review and practice problems. The main reason for my academic decline the past year was due to my lack of attendance due to my medical conditions. To prevent this from happening again, I now have to get a blood test every few weeks and have meetings with my family doctor until the stomach bacteria is completely gone and my liver, which has shown a decline in function returns to normal levels. During my last meeting with family doctor, he said that I have now recovered enough to have a normal course load, however, I will need to still have blood tests and meetings to ensure that the condition does not come back. New Information: I have the doctors note that I have gotten now that states that I have now recovered and am able to complete academic requirements. I have older documentation as well which is a doctor's note from when I was sick during the examination period and the medications that I was given. I was also given xrays and throat swabs but the clinic that I got them at is not providing me with documentation.
