Petition Letter: Dear Associate Dean,

Please accept this letter as my formal request to waive the 21-day deadline for an academic appeal I wish to submit for the 2014-2015 year. In late October 2014 I experienced a sudden personal loss resulting in unmanageable levels of distress. My struggle to cope with this crisis made it difficult to carry out basic every day tasks, let alone engage in my academic activities. What is especially relevant to this deadline appeal is that I did not have even a vague timeline for how long it may take to recuperate from this situation.

I have included documentation from my doctor in the form of a letter and a completed Off-campus Physician¿¿s Note. These documents also address information needed for my academic appeal as the Arts and Science office has indicated the same documents may be used for both the 21-day deadline and academic appeals.

More detailed information can be made available if needed to support this appeal.

Petition Letter: Dear Associate Dean (Studies),

Please accept this letter as my formal request to late drop ANAT315 from the 2014 Fall term and PHGY214 from the 2014/2015 Fall/Winter term. In late October 2014 I experienced a sudden personal loss resulting in unmanageable levels of distress. My struggle to cope with this crisis made it difficult to carry out basic every day tasks, let alone engage in my academic activities. My appeal includes supporting documentation in the form of a letter and a completed Off-campus Physician¿¿s Note from my doctor.

Although I am still occasionally managing distress from last fall¿¿s events, it is not nearly to the same extent as I had previously experienced. Because the period of shock has passed and I have significant professional supports in place, I am better able to handle my academic responsibilities.

A reduced course load will also contribute to my academic success in my final year at Queen¿¿s. With only 12 units remaining in my degree program, I am able to work towards completing 6 units each term.
