Petition Letter: My name is Trevor Nanton, my student number is 10110218 and I am appealing to drop Math 126 from Winter (2nd) semester 2015.
During the first semester of 2014 I began dealing with extenuating circumstances that severely affected my mental state. These events began around the end of October 2014 and continued past the date of the final exams of first semester. During this time, I had gone to consult a local doctor from the CDK Family Medicine and Walk-In Clinic, who after hearing my situation had prescribed me Anti-Depressant medication and also gave me a three week exemption from all academic evaluation, as well as an exemption from any academic evaluation done in the week prior to my visit. Following these weeks my condition hadn¿¿t improved, in light of this my parents decided it was best to bring me home from Kingston for a week. After returning to Kingston I went to the FCMHAS (Frontenac Community Mental Health & Addiction Services) and consulted their in house teams as well as their in house psychiatrist. After meeting with the psychiatrist he had decided it was best for me to return home for the remainder of the term. He had also prescribed me a new medication which he told me was meant to help with psychosis as well as anxiety. His team then got in touch with Disability Services at Queens and had worked with them and me on deferring my first semester exams.
During the period when these events were occurring, I found it extremely difficult to focus on my academics. When I tried to study alone outside of my lectures, I would start breaking down. When I attended lectures during this period, I found myself having to leave because I couldn't handle the thoughts going through my head. When I was at this point, these sort of break downs weren't limited to my academics, rather the breakdowns would occur whenever I was left alone with myself or with my thoughts.
Following the events of first semester I submitted an Academic Appeal for my courses ECON 250 as well as for POLS 243 as I was unable to successfully participate in and complete those courses (which were fall semester courses) in the state that I was in. In spite of this, I decided to try to continue taking part in my full year calculus course MATH 126 with the hope that I would be able to recover and improve my academic standing in this class as well as in the my other courses second semester. Unfortunately though, I was unable to bring my mark up enough from where it was following the fall semester. At the time of my previous appeal, I did not know my mid-year mark for MATH 126 so I was acting under the assumption that it would be salvageable. I was still left unaware of my mark even toward the end of the winter semester (I will attach a screenshot of my Moodle marks page taken on April 23 2015 that shows that my Mid-Year mark still not updated at that time.
This second semester was an extremely busy time for me. I have spent it trying to recover my marks and catch up in my courses work loads which suffered in my first semester this year. My mental state has improved drastically over the second semester. I have been working on this with FCMHAS and they have finally released me from their care during the latter half of second semester (they had been treating my Psychosis as well as my Trauma symptoms). I am also currently taking a summer course for computer programming in order to recover credits that I have fallen behind on over this year, and hopefully I will be able to raise my average in the process.
Should my appeal be granted, I would very much like to keep improving and continuing on this course. Since the start of my recovery I have been making positive progress both in my everyday life as well as academically. I understand the importance and value of what Queens has to offer and I really want to be successful in my academic pursuit and beyond. With the ongoing support of Disability services, my family and Queens support services I am confident that I will be able to turn my situation around.

New Information: I was unable to obtain a reference letter from FCMHAS due to the fact that I wasn't able to get to Kingston to sign a release form and meet with the Psychiatrist again as I am working up north this summer. Though I submitted a letter from them with my previous appeal earlier this year on January 20 which can ideally be used as reference in this case. Sorry for the inconvenience.
