Submit an Academic Petition or Appeal

Petition Letter: Due to physical illness, cancer, and other health issues I had during the course. I was in contact with the professor; an incomplete grade was to be placed on my transcript. Was an F entered for my grade if the grade was never entered? My final exam was deferred, and Kate Nelson, Exams, coordinated assignments. I could not postpone the exam further, even though I still experienced significant health issues. During the exam, I experienced technical and health problems. Frustration and not feeling well, I just completed to get over with. I wrote to everyone about it and said once all grades were posted. I could review options for appeal at that time. The assignment was not marked, and the exam wasn't graded. I didn't return to fall term due to medical conditions.

I return to the Queen in Jan 2023. I inquired with QSAS advisor Carlone about options as the professor is no longer with Queen. She retired. Caroline Teske reached out to Alana Korczynski on Jan 30th, 2023 was able to confirm that someone could mark my exam and assignment if I could locate them. I was asked to verify how the missing assignments was submitted o the W22 onQ page. Some were submitted through OnQ, and reminders were emailed the assessment to the course email. Throughout the process of working through fixing my grade, I had to withdraw again from winter term due to health problems, cancer treatments, and sugary and boken rights dominate had. I uploaded medical documents to the QASA return portal. Due to the extenuating circumstances beyond my control due to my physical illness and the psychological challenge, I am requesting for review and consideration grade of CR for the courses.
