Submit an Academic Petition or Appeal

Petition Letter: My name is Kelly Mifflin and I was enrolled into PHGY 170 and PHAR 100 for the fall 2023 term while managing my part-time work schedule as a paramedic. Unfortunately, I was physically assaulted on August 26, 2023, resulting in my being diagnosed with a concussion on August 28, 2023. My physician has since referred me to physiotherapy directed at concussion therapy due to my long-term symptoms, where I was diagnosed with post-concussion syndrome on November 24, 2023.
I have been having great difficulties in school this term and feel it would be beneficial for me to drop out of PHGY 170, as the grades I was receiving are not reflective of my character or competence. I would be grateful for the opportunity to redo this course upon my full recovery.
I believed that the deadline to drop out without penalty was October 31st, and was extremely disappointed upon realizing I missed the deadline by one day. I apologize for any delay in providing information about my circumstances, as I have not been myself for the last few months.
