New Information: I was unable to obtain a reference letter from FCMHAS due to the fact that I wasn't able to get to Kingston to sign a release form and meet with the Psychiatrist again as I am working up north this summer. Though I submitted a letter from them with my previous appeal earlier this year on January 20 which can ideally be used as reference in this case. Sorry for the inconvenience.
