Submit an Academic Petition or Appeal

Petition Letter: Dear Associate Director of the Health Sciences Program,

I am writing in regards to the Summer 2022 semester for the course Social and Physical Determinants of Health and Disease (GLPH 171) taught by Dr.Colleen Davison I am looking to obtain a late drop for this course due to some additional extenuating circumstances faced this semester that were unexpected.

I was advised on August 1st by my professor to consider reaching out to an academic professor to late drop the course. I did not drop the course by the drop deadline of June 24th as I was not expecting for my health to continue to decline. I previously obtained academic accommodations for an accident I was faced with in November 2021 and was hoping no other health issues would impede me from obtaining my degree progression. My surgery was July 5th, 2022 and has proven to be more difficult than my surgeon and I anticipated accompanied by chronic pain that has the most likely diagnosis of Chronic Regional Pain Syndrome at this point.

My healthcare team and I are continuing to pursue diagnostic testing to help me be as comfortable as possible.

I currently have an IN in HSCI 190 but, the professor has extended academic deadlines and deferred my exams in order to ensure success within the course.

I am pursuing documentation that supports the information above, my main healthcare provider is currently on vacation, I have included the last visit with my surgeon and can provide more recent documentation as well. I would greatly appreciate a late drop of GLPH 171 and the opportunity to re-take this in the Fall 2022 semester so I can continue my progression into a degree program at Queen’s University.
