Petition Letter: Letter of Personal Experience to Petition for Deadline Waiver

To Dr. Jenn Stephenson, Associate Dean of the Faculty of Arts and Sciences:

My name is Adam Carbonara, and I am a fourth-year student of the life sciences specialization stream. I write to you today concerning the deadline for credit standing (CS) applications for completed courses. At this time, I am petitioning to waive the deadline to apply for CS in courses I took throughout my second year.
It was made apparent to me as of recently that a part of academic success services ranges beyond accommodation inquiries and requests for short-term leaves. This knowledge was imparted on me by my mental health counsellor, whom I had been placed with upon inquiring for mental health concerns. My mental health counsellor educated me on the academic appeal procedure and CS, where I could have an opportunity to justify the extenuating circumstances which occurred that caused me to achieve lower grades. This counsellor helped guide me through the steps involved, and I credit them for pushing me this far as it gives me a better hope to improve my mental health regarding my education. Prior to this meeting, I had no previous knowledge of the procedures to apply for CS.
To be elaborated on further in my formal petition for CS, should I be granted the opportunity, I suffered some extreme circumstances in my second year which completely shifted my focus away from my education, causing sub-average results. Since then, my mental health has declined due to the fact that I wish to continue a future of graduate studies and pursuing medical school, all of which will be extremely difficult (if even viable) in my current situation. Upon learning of this petition process, I have restored some hope that I can continue my pursuits by adjusting my transcript to what is justified relative to my actual academic progress.
If I am granted the opportunity to file a petition for CS, I plan on submitting supporting legal documentation that encompasses the duration of the circumstances I faced, along with another hand-written letter of experience further detailing my experiences, and an explanation for each course that I wish to inquire about for CS. I have all documentation ready, and awaiting the outcome of this petition. Along with this document is a letter of support from my counsellor, who again recommended me to take this opportunity. I plan on using my legal documents to support the timeline of my request in applying for CS in my second year courses.
I make all claims in this process based on nothing else except for pure integrity and honesty. I appreciate that support like this exists and wish to keep an honest standard of communication to justify a righteous result that would benefit my present and future career. At this point, I am well educated on the services and deadlines for academic considerations, and plan to make this my only petition as I am aware of course drop deadlines and other dates for appeals.
