Submit an Academic Petition or Appeal

New Information: The interface for submitting supporting documentation does nothing when I select the upload file option. I have supporting documentation from my primary care provider that I am not able to submit with my petition. I will be sending the supporting documentation over email, please let me know if you require anything else.
