Dear whomever this may concern,

My name is Taylyr Viegas and I’m writing this petition regarding the mandatory 12
month leave from my education at Queens University. My rationale is during the last 6 months of
my education, I have been struggling with an ongoing illness that is still being dealt with
currently.

Attached I have a letter written by my physician in Ancaster, Dr. Yasinian. I also have
medical records from my multiple visits to the CDK family clinic in Kingston, medical records
from Dr. Yasinian, referral records, life labs blood clinic records as well as KGH hospital records
from all my visits throughout the past 6 months. If these listed items/records are needed I can
provide them upon request if deemed necessary. My ongoing health issues, that resulted in
numerous medical appointments, hospital emergency trips and various medications prescribed
subsequently impacted my studies resulting in my poor grades the second semester.

To achieve my goals and succeed at Queens, I have written an academic plan for
guidance on my journey throughout the next few years. I have planned with my family to
participate in tutoring and seek help from the services provided by Queens. My father is a Band
Member of the Six Nations of the Grand River, and I will be reaching out and utilizing the
indigenous services that are available to me at Queens University as part of my plan.

Please consider my petition as I am extremely prepared to do whatever it takes to move
forward with my education and studies at Queens University. With the multiple supports Queens
provides, I will utilize them as well as seeking help from upper years, professors, and
extracurricular clubs. I am extremely confident that my proposed plan coupled with the supports
in place and the improvement in my health will lead me to a successful year at Queens. Please let
me know what steps I need to take to progress with this motion and continue my education this
upcoming school-year.

Sincerely,

Taylyr Viegas

will benefit me in learning more about the major I want to pursue.

Motivation

Originally, I wanted to go into psych, although as the year progressed, I found Global
Development to be much more interesting encouraging me to work and learn within this degree.
I have also learned quite a lot from Employment relations as I’m working in HR over the
summer. I would like to minor in employment relations if given the opportunity.

Personal / Financial

I am not struggling with any mental health related issues; I am also financially supported and
face no stress within this category.

Situational Demands

The situational demand I faced this year during second semester dealing with a constant illness
that left me exhausted and weak that I am still dealing with and I am continuing to receive
treatment and I am improving day by day. As stated in my petition letter, I have been to multiple
doctors’ appointments with my physician in Ancaster and a physician in Kingston. I have also
had blood taken on multiple occasions, have had referrals to specialists, hospital emergency trips,
an ECG, multiple prescriptions and I am now awaiting my appointment with a specialist. I have
all these records and can provide them if needed, as well Dr. Yasinian my family doctor has
advised she would be more then willing to provide any further information should you require it.
As a result, it has been severely difficult and stressful trying to keep up with my academics
during this period (past 6 months) while being challenged with severe health issues. This
summer I am scheduled to see a specialist regarding my current illness and will have this dealt
with to be ready for the next year of my education at Queens’ University. I continue to recover
and am building back my health.

Health Plan

a) Receive the proper medication, or possible surgery from a specialist over the summer.
b) Daily walk / exercise to increase my health.
c) Taking vitamins or medicines provided by a healthcare professional to help increase my
immunity to illness/build back my health.

My Overall Goals

1. Get my illness resolved.
2. Go into Global Development as my major.
3. Take any additional courses needed to accomplish my goal.
4. Ask for help from multiple resources during my academic schoolyear.
5. Pass all classes with a B or higher.
