To whom it may concern,
I am re submitting my application to change two classes from a mark to a credit. As I am being to apply for my graduation process I realized that my letter mailed last year was never received by the university and thus am having to re send it. My credit card was never charged so I can only assume that my letter was never received in the mail. Below is the personal letter that I sent along with copies of the original paper work that was included. I did notice that the forms have changed slightly since then but unfortunately this is all that I have from then.


Dear Arts and Science committee 06/06/13

Thank you for taking the time to read my letter along with my supplemental documents. I am appealing to have two of my classes changed from a mark to a credit standing as suggested by my doctor and counselor. I am also applying to wave the 21 day appeal deadline as it did not allow me enough time to meet with my main health care physician and acquire all the documentation needed for a successful appeal.

I have been diagnosed with depression for the past few years. It has affected my academic success before and unfortunately it appears to have done so again. During the winter term I struggled with my classes and sought out my professors to ask for assistance. Although not all replied to my emails, the professor that did (phys p16) did try and work with me allowing me one on one help and extra time for class work when needed. I have also done an intake assessment at the providence care centre as suggested by my doctor. After my initial intake it was clear that more work was required with my case. I had also been meeting with the Associate Dean of Arts and Science to work on my academic success but as those meetings decreased so did my ability to receive help for my schooling due to my severe depression. During the winter term I overdosed on pain killers twice and was treated by Hotel Dieu Hospital. After my overdose the decision was made that I was to move back to Vancouver, live at home with full time supervision, and continue my studies via online classes to ensure that my mental health problems could be taken care of. I have struggled with this for years and am exhausted as to the toll it has taken on my academic career at Queen¿¿s. I do not want my health conditions to affect my ability to achieve greatness in my academic career. I know I am able to do more than what I have suffered through and hope that my marks given at a time of great need and struggle do not hold me back from working on a future that I am more in control of. For this reason I am appealing to have my classes changed to a credit standing. The professors that I was able to reach support me in this decision and have been very helpful in assisting me. My doctor as also provided documentation to support my request and has encouraged me to continue with this appeal.

Thank you for your understanding and consideration during this difficult personal time. I strive to do better in the future and make this school proud of its students despite there health conditions and efforts to move forward.
