Petition Letter: Hello,
I am kindly requesting an exam deferral and incomplete (IN) grade for BIOL 103, as well as a credit standing (CR) for PHGY 170, due to extenuating circumstances beyond my control that have significantly impacted my academic performance.
Since the beginning of 2024, I have experienced a serious exacerbation of disability-related symptoms, which have been further complicated by challenges with managing my treatment and medications. These circumstances have affected me over the course of a year, during which time I have faced considerable difficulty with cognitive functioning, executive dysfunction, mental illness, and overall symptom management. As a result, I was unable to complete the coursework to the standard required to achieve my best performance on assignments, and I was also unable to adequately prepare for the final exams in both PHGY 170 and BIOL 103.
Throughout this period, I have consulted regularly with my healthcare team to adjust my treatment plan and manage symptoms, and I have remained in communication with my QSAS advisor to seek appropriate academic accommodations. Despite these efforts, the severity and unpredictability of my symptoms continued to interfere with my ability to meet academic deadlines.
In light of these circumstances, I am requesting an exam deferral and an incomplete (IN) grade for BIOL 103 to allow additional time to complete the final course requirement, and a credit standing (CR) for PHGY 170, as my ability to complete the course to the best of my academic ability was significantly impaired.
