Submit an Academic Petition or Appeal

Petition Letter: Dear Associate Dean,

I am writing to request an extension for a deferred examination scheduled for BIOL 200 earlier in May, which I was approved to defer due to a registered mental health condition on file with QSAS.

Unfortunately, despite my continued efforts to manage my condition and prepare for the deferred exam, my mental health worsened, impacting my ability to adequately study and perform to the best of my academic ability. I have been in communication with my QSAS advisor and the course professor and have been doing better lately.

Given these circumstances, I kindly ask for an additional extension for the deferred exam. I remain committed to completing my academic responsibilities and am requesting this additional time to ensure I can complete my requirements for the course after my commitment throughout the term.

I appreciate your time and understanding, and I hope for your consideration of this matter.
