Petition Letter: To whom this may concern,
My name is Alex I am writing to respectfully request consideration for a CR standing in Math 121 due to serious extenuating circumstances that significantly affected my academic performance especially leading into the during the final exam season. However, this was happening throughout the semester too because of the gradings.
Throughout the term, I experienced a distressing issue with my course grades, which were repeatedly scrambled or incorrectly displayed on my OnQ. These inconsistencies caused me confusion and distress, as I was unable to accurately track my progress and understand how I was preforming and where my standing was. This lead to a series of Anxiety responses including rapid panic attacks, vomiting, migraines, dizziness, and consistent nausea. These symptoms caused me to go to the Kingston hospital one night during the final season, due to the symptoms extreme behaviour. And eventually became medically documented and is outlined in the attached note from Dr Melanie Henry, my physician.
The impact of this situation was strongest in the days leading up to the final exam. Although my grades were eventually corrected, this happened only shortly before the exam I am still unsure if there was a technical issue or an incorrect marking. Still by this point the damage to my mental health, study habits, and exam readiness had already taken a toll. The uncertainty and stress substantially disrupted my ability to prepare effectively, concentrate, and preform to my best ability especially on the final exam.
The documented issues which were beyond my control and had a direct, negative effect on my final exam performance I am requesting to be granted a CR standing for MATH-121. I hope this will allow the transcript to show my commitment to the course while also acknowledging the medical issues I faced.
Thank you for your time and understanding in reviewing my petition.

Alex Daicoff
