Petition Letter

Petition Letter: To Whom It May Concern,

I am a second year Art History and English student in the faculty of Arts and Science. On March 9th I sustained a head injury that resulted in a concussion, requiring me to take 2 weeks of cognitive rest at clinical advisement. This cognitive rest restricted me from schoolwork and viewing screens, which severely affected my ability to preform academically. I contacted my Disability Services advisor Jeanette Parsons whom on my behalf contacted my instructors to make arrangements on outstanding papers for completion prior to the exam period. During my examination studies, on April 17th I became nauseous, dizzy and disoriented, forcing me to miss my ARTH 260 exam and seek medical help at Kingston General Hospital. There I was told I has post concussive syndrome and was to rest once more cognitively for two weeks. I then inquired about medical documentation from a walk in clinic in the Kingston area. Concerned about my health and its affecting my academic performance, I booked an appointment with Dr. Lacoursiere for a more thorough assessment. She provided me with a verification of illness document and referred me specialist Dr. O'Conner at the Queen's Sports Medicine Clinic. He assessed that I had remaining symptoms of the concussion and needed to avoid severe cognitive stress for the near future. With dIalogue from my instructor Dr. Hoeniger and Ms. Sue Blake, Assistant Dean of Studies and Student Services, it was decided that I should complete my outstanding essay by September 1st to bring my course total to 70, whereby I could apply for an aegrotat standing to receive a final grade in the course. Dr. Hoeniger will be on sabattical from July 1st- December 31st which makes organizing the sitting and marking of an examination for the course very difficult.

Through this letter, it is established that my academic performance was hindered from my physical illness and my ability to sit my examination and complete this course in standard terms was a direct result of my recurrent concussive symptoms.
