Submit an Academic Petition or Appeal

Petition Letter: To the Faculty Office,
I am writing to formally petition for a grade reassessment in CHEM 323 based on extenuating circumstances that significantly impacted my academic performance throughout the semester and during the final exam period.
I have been managing an ongoing mental health condition that affects my concentration and academic performance. Throughout the term, my ability to perform consistently in CHEM 323 was compromised due to fluctuations in my treatment. Side effects from medication changes and gaps in my prescription interfered with my focus, retention, and productivity, which were reflected in my coursework and assessment results during the semester.
These challenges culminated during the final exam period. Although I deferred the original exam in the hope that I would be in a more stable condition, I faced an unexpected disruption in my treatment during the lead-up to the deferred exam. As confirmed by my physician, Dr. Meagan Thang, I was not on my prescribed medication at the time of writing the exam and was experiencing medication-related side effects. This made it extremely difficult to prepare effectively or perform to the best of my abilities.
Despite doing my best to manage these challenges, including continuing to study and remain engaged in the course, my cognitive function was significantly impacted. I was mentally foggy, easily fatigued, and unable to apply the concepts I had studied with clarity and confidence. As a result, my final exam performance does not reflect my true understanding of the material or the effort I invested over the term.
I respectfully request that my final exam grade be reconsidered in light of these extenuating circumstances, or that I be given an opportunity to demonstrate my knowledge in an alternative format. I believe these events were beyond my control and had a substantial impact on my academic performance in CHEM 323.
